This obviously wouldn't work against a person with narcissistic personality disorder.

if you were to remove them from their job you could have a lawsuit on your hands. If you "punish" someone for a disease or disability your gonna have even the worst of lawyers breathing down your back. If he didn't have health issues would you still be changing his work? Will a court see it that way?

You generally speaking can't sack someone for being ill. You can however sack them if they suck at their job. Your particular case sounds like the latter.
